New Patients

We do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance or medical condition. We accept requests to join the list from eligible people who live in the practice area.

To register as a new patient, please:
- Check that you live within our practice boundary 
- Register Online with a GP Surgery
- Register with a GP with a Paper Form 

We would recommend you register with us online as it will be much quicker and convenient. Usually when you apply online, there will be no need to provide proof of address or immigration status, ID or an NHS number. This is because the service is run by the NHS, so your personal information is safe. 

If you are unable to complete an online application, you can complete a GMS-1 form. Please hand in the completed forms to one of our reception teams with proof of address. Photo ID will help us to speed up the process and request your medical records. 

When you become a registered patient, you will be assigned a named GP. This is provide you with more consistent and personalised care.

Should you have any chronic diseases and / or regular medications i.e. diabetes, asthma, ischaemic heart disease etc, you will be advised to make an appointment to see our health care assistant for a new patient check.

What is the purpose of a named GP?

A Named GP is the doctor responsible for managing your overall care. They will:

- Have better understanding of your medical history.

- Oversee your treatment plans and long-term health.

- Provide more continuity in your care.

Will you still see others doctors?

While your Named GP will be your main doctor, you can still be seen by other GPs and clinicians when needed. If your Named GP is unavailable and you have more urgent (acute) issues, you may need to see another GP or appropriate clinician. We will always try to accommodate you seeing your named GP where possible.

How to book an appointment:

When you are booking an appointment, our reception team will:

1. Confirm your full name and date of birth. 

2. Check who is your Named GP and book your appointment with them whenever possible.

3. If your Named GP is not available, an alternative GP can see you for urgent issues.

Additional Requests

If you have specific requirements for example, requesting a female GP, we will do our best to accommodate your needs. Occasionally the reception team may recommend you seeing a different GP or clinician to better suit your appointment requirements. For example, seeing our in-house physio for muscular issues.

 

Medical treatment is available from the date of registration.
